This agreement is between the purchaser and Cody Air Conditioning & Heating, Inc., for the purpose of providing
non-warranty maintenance on the heating and air conditioning system(s) at the above address.  This agreement may
be terminated by either party without cause by rendering a thirty-day written notice.

SEMI-ANNUAL AGREEMENT
Ÿ          One (1) filter change plus complete inspection of the heating system(s)
Ÿ          One (1) filter change plus complete inspection of the cooling system(s)
Ÿ          10% discount on repairs, parts and accessories during the lifetime of the agreement The work includes
two (2) visits during the 12 month period while the agreement is in effect.  The visits are scheduled at
periodic intervals by Cody Air Conditioning during regular office hours.
